Overview

Various staff and faculty at the college have personal extensions and from time to time may change departments or retire. Use this service if you are taking over a pre-existing extension that was assigned to an individual, not a department or service.

Many phones have a primary extension associated with it, typically a room number (e.g. RM B333). This is to assist with management of phones and will not be changed. As employees are moved around, their extensions will be added or removed as secondary extensions to the phone.

Before you make request

Please ensure you have the following information ready when filling out the form:

  • Extension (e.g. 4321)
  • Previous persons name associated with extension
  • Primary extension of phone(s) the extension is associated with
  • Is there an associated voicemail that needs to be updated?

After you make request

You will receive an email notification from the email system with a ticket number for reference. You may reply to the email if you have any further details or questions.
Upon completion of the request, you will receive an email from IT requesting that you confirm all is as requested.
